Feasibility and surgical technique of percutaneous transforaminal discectomy for the treatment of lumbar disc herniation

Percutaneous transforaminal endoscopic discectomy (PTED) is an alternative minimally invasive technique for the treatment of lumbosacral radicular syndrome resulting from lumbar disc herniation. The latter performed with help local anesthesia and sedation in awake patient advantage direct feedback. Access to target lateral follows a safe trajectory through Kambin triangle. main advantages this surgical are possibility early discharge within 2 hours after its completion, negligible blood loss, lower risk infectious complications, convenience patients obesity, reduced epidural fibrosis. There currently no large randomized trials good quality demonstrating PTED over open interlaminar access established practice. In worst case, not known pose additional risks patients. operative clinical case discussion about applicability new approach herniations presented.
